pertaining to Recital 2021 at Queens University on May 28th & 29th
As with the transmission of any communicable disease like a cold or the flu, you may be exposed to COVID-19, also known as “Coronavirus,” at any time or in any place. Be assured that we have been made aware that Queen’s University has followed all state and federal regulations and continues to take daily precautions in cleaning, social distancing, and regulating activity on campus to limit transmission on their campus. On top of this, they have implemented several additional regulations specific to Queens to ensure the safety of their campus and community. Those regulations are as follows:
Failure to abide by any of these practices will result in termination of our contract with Queens and the immediate departure from campus.
Despite careful attention to sterilization, disinfection, and use of personal barriers, there is still a chance that you could be exposed to an illness on Queens Campus, just as you might be at your gym, grocery store, or favorite restaurant.
By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 while on Queens University Campus and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I voluntarily agree to assume all the foregoing risks and accept responsibility for any injury to myself and any event participants. I hereby release, covenant not to sue, discharge, and hold harmless the University, its employees, and representatives, as well as Havilah Dance Company, its employees, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto and thereby consent to abiding by all policies and regulations set in place to prevent transmission while participating in this external event hosted by Havilah Dance Company.
